The manufacturing, testing, release and distribution of vaccines is a complex and multifactorial process with hundreds of steps, requiring a diverse pool of human resources. For vaccines to result in actual vaccination of people, highly coordinated efforts are required from international supply chain, manufacturers, regulators and State and Central government agencies.

Production scale-up of vaccines is a step-by-step process, involving several regulatory SOPs of GMP (Standard Operating Procedures of Good Manufacturing Practices). There is a four-month lag time for COVAXIN® to translate into actual vaccination.

The timeline for manufacturing, testing and release for a batch of COVAXIN® is approximately 120 days, depending on the technology framework and regulatory guidelines to be met. Thus, production batches of COVAXIN® that were initiated during March this year will be ready for supply only during the month of June.

Based on Central Drugs Standard Control Organisation(CDSCO) guidelines, all vaccines supplied in India are mandated by law to be submitted for testing and release to the Central Drugs Laboratory, Government of India. All batches of vaccines supplied to State and Central Governments are based on the allocation framework received from the Government of India.

The timeline for vaccine supplies to reach the depots of the State and Central Governments from Bharat Biotech’s facilities is around two days. The vaccines received at these depots have to be further distributed by the State Governments to various districts within their respective states. This requires additional number of days. Pandemic vaccines are distributed by respective governments equitably across all sections of the population. Vaccines once available at the vaccination centers are then administered to recipients over a period of time, based on demand.

“Prevention and Self Care of Arthritis Key to Bring Down Healthcare Burden During Pandemic”: Expert

Bengaluru, May 28 2021: “With prevalence higher than that of diabetes or cancer, arthritis has emerged as one of the widely spreading diseases in India, currently affecting nearly 200 million or about 15% of the population. Preventive measures and proper self-care are important for the country to bring down the burden from the disease, and to make a big difference to patients especially during the ongoing pandemic, when getting in-person consultation or medical intervention is proving to be difficult,” said Dr. Chirag Thonse, Consultant Orthopedic Surgeon, Vikram Hospital.

In a press release issued today, in connection with the Arthritis Awareness Month, being observed in May every year, across the world, Dr Thonse said that arthritis is a swelling or tenderness in one (or more) of joints. It can affect many different parts of the joint and nearly every joint in the body. The most common forms of arthritis are osteoarthritis, rheumatoid arthritis, gout, and ankylosing spondylitis. Arthritis affects people in different ways. But the most common symptoms are pain, stiffness or reduced movement, swelling, redness, and warmth of joints. Also, the general symptoms include tiredness, weight loss, and feeling unwell.

Dr Thonse said that many people think arthritis is a normal part of getting older. “However, this is not true. It can affect people of all backgrounds and age groups. Anyone including children and young people can get arthritis. Two out of every three people with arthritis are in the age group of 15 to 60 years,” he pointed out.  Stressing on the importance of early detection, Dr Thonse said, “Not many think that it’s a serious medical condition, and it needs timely medication and care so that it doesn’t aggravate further. Delay is often due to lack of awareness. Talk to your doctor if you have pain and stiffness associated with swelling, redness and warmth of your joints that begins with no apparent reason and lasts for more than a few days.”

On useful preventive measures, Dr Thonse said that diet and exercise can help prevent the incidence and progression of arthritis. Maintaining a healthy weight and engaging in physical activity reduces the risk of developing arthritis, especially osteoarthritis. Overweight women are almost four times more likely to get knee osteoarthritis than women with normal body weight. 

“Physical activity can ease arthritic pain, improve function and mood, and delay the onset of disability. Even small amounts of weight loss have proven to reduce pressure on the joints significantly,” he said. “Consult your doctor or physiotherapist about the type of activity such as cycling, dancing, walking, gardening, swimming, and yoga that would be most appropriate for your condition. Exercise is crucial. Just 30 minutes of exercise five times a week helps joints stay mobile and strengthens the muscles that support your knees and hips. It’s essential to focus on low-impact activities like walking, cycling or swimming. Gentle stretching can improve your range of motion and keep your joints healthy. Try to work on simple stretches every day.” 

Talking about the role of a healthy and balanced diet in prevention of arthritis, Dr Thonse said that various nutrients may help boost joint health and reduce inflammation. Foods that may help delay the onset or progression of arthritis are fruits and vegetables that provide antioxidants, low-fat dairy foods that contain calcium and vitamin D, and healthy oils such as extra virgin olive oil. These foods are a part of an anti-inflammatory diet. In contrast, foods that can increase the risk of oxidative stress, and inflammation are highly processed foods and foods that contain added sugar or salt, unhealthy fats such as trans fats and saturated fats, and red meat. Hence these foods should be avoided. Keeping blood sugar under control is also important, as high blood sugar can stiffen the tissue that supports joints and make them more sensitive to stress. 

Emphasizing the need for avoiding injuries, Dr Thonse said that an injured joint is more likely to develop arthritis than one that is never injured. “Wear protective gear when playing sports and protect your joints from everyday strain using the correct technique when sitting, working, and lifting. For example, lift with your knees and hips, not your back – when picking up objects.” 

Dr Thonse called for the need for the public in making healthy lifestyle choices such as not smoking, avoiding stress, getting adequate sleep, and reducing alcohol intake can improve your well-being by keeping you high on emotional wellness, self-management and motivation.

Covid Can Impact the Eyes

Bangalore, May 27,2021: The first case of COVID reported was by an eye doctor in Wuhan China. While COVID is a respiratory disease there are many eye conditions where the effects of Covid may be seen.

One of the complications of COVID is the presence of occurrence of blood clots. These blood clots can be found in small medium as well as big blood vessels that is why AI in treatment of Covid doctors usually add on an anti-clotting medicine.

As far as the eyes are concerned tiny blood clots may be found in the retina which is the tissue at the back of the eyeball. The patients may or may not notice this. If noticed, the patient may find that the vision in one eye is a bit less or substantially less. In such a situation it is important that the patient reaches his nearest eye doctor at the earliest for proper treatment.

Another side effect of Covid on the eye maybe inflammation of the inner parts of the eye called uveitis and retinitis. It must be remembered that these are rare conditions. Covid generally affects the lungs and to a lesser extent the gastrointestinal system.

The role of steroids is very Central to the treatment of Covid complications. While in the initial stages of viral Covid infection the steroids can play very harmful role, especially in the first week of the disease, the same steroid medications can be lifesaving to the patient in the presence of a complication called cytokines storm. The topic of black fungus is very prominent now a days. Mucormycosis is a fungal infection that does not affect most normal people. It affects very few people and is an extremely rare disorder even today. However, after Covid infection one category of patients are at higher risk for the Black fungus. These are patients who have suffered severe lung damage and as a result would have breathe oxygen through the nasal tubes. Those patients on ventilators and those who have taken steroid tablets and those who’s diabetes is not well controlled only such patients who have all three above listed conditions in them are at increased risk of black fungus.

One need not fear or spread panic with respect to this fungal infection. It is important and adequate that patients who have any pain or trouble in their eyes or sinus or central nervous system visit their family doctor very early with their symptoms and make sure that all is well.

Compared to the total number of non-Covid patients along with Covid patients not known to the government the percentage incidence of black fungus is extremely low. While statistics are not easily available on this topic, it is my guess of estimate that the incidents of black fungus is less than 0.01 percentage.

So as a country and as people we await for the pandemic to subside, it is sufficient if we be alert follow all the safety guidelines and keep in close touch with your family doctor at the beginning of any unusual or abnormal symptoms.
