Doctors at Sakra World Hospital does miraculous Neuro-rehabilitation of 36- year-old with severe head injury following bike accident in US

January 18, 2023: Not all hopes die if you know the way out and believe in miracles. When 36-year-old Mr. Pravin Raj Radha met with a fatal accident in the US after he fell from his bicycle in May 2022, he suffered severe brain, facial and eye injuries which resulted in leaving him at a comatose state and losing all hopes of his survival. His condition was treated immediately in one of the top hospitals in the US, undergoing intensive care where the US doctors estimated less than 1% chance of survival. Pravin shared his story of getting a new lease of life today at a press conference in the presence of Mr Yuchi Nagao, MD at Sakra World Hospital, Mr Naoya Matsumi, DMD at Sakra World Hospital and Mr Lovekesh Phasu, COO at Sakra World Hospital, and Dr Maheshwarappa 8 M, Head and Senior Consultant for Neuro Rehabilitation at Sakra World Hospital as well as Pravin’s treating doctor in India.

Despite being on a ventilator in the ICU in the US for 3-4 weeks, Pravin’s health never improved. Rather than giving up and taking Pravin off the ventilator, he was flown carefully to India by chartered flight with ICU team and equipment. After consulting in other hospitals, Pravin was referred to Dr. Maheshwarappa BM, Head and Senior Consultant for Neuro Rehabilitation at Sakra World Hospital, one of the top hospitals in Bangalore. He immediately assessed Pravin’s complex traumatic brain injury, severity of disability and medical challenges, loss of cognitive and behavioral skills and immobility. Following his recommendations for rigorous and comprehensive neuro rehabilitation using advanced technologies, a miraculous recovery and successful discharge took place five months later.

Pravin was brought to the hospital in a comatose and bedridden condition, with weakness of both upper and lower limbs, agitation, and difficulty with sitting, walking, speaking, and swallowing. Sakra Neurorehabilitation specialists and professionals included Physiatrists, Neuro-Physiotherapists, Occupational Therapists, Clinical Nutritionists, specialized Rehabilitation nurses, Clinical Orthotists, and Neuropsychologists. The use of advanced rehabilitation technology included Hyperbaric Oxygen Therapy(HBOT), Robotic Assisted Gait and Arm Training, wearable sensor based therapies, virtual reality clinical modules, augmented reality gamification, Pneumatic Neuro-gym and digital Balance and Coordination platforms.

Dr Maheshwarappa B M explained that “Neurorehabilitation plays a significant role in the healing process of those who have suffered traumatic brain injuries. The Rehab program is designed based on the patient’s condition and needs to help them recover from the arduous effects of the injury. Pravin was brought to us in the most challenging situation with almost all hopes of his recovery dying down. But for us, after assessing his condition, not all hopes were lost. We developed the rehabilitation program at Sakra Neurorehabilitation Centre to address his core problems, utilizing advanced rehabilitation equipment and scientifically enhanced infrastructure.”

A 3- to 4-month intensive neuro rehabilitation program helped Pravin come out of a coma and begin a steady recovery process. In addition to muscle strength exercises, Pravin also received multi-sensory stimulation (auditory cues, tapping, music therapy) to regulate his low arousal levels. Over time and with self-determination, he has been able to walk independently with good balance. For his cognitive and perceptual skill improvement, occupational therapies / involved identification of objects, participation in sports activities in groups, vocational activities like writing, using the keyboard, using a mobile handset, dressing himself independently, bladder and bowel management, bathing. His family members were totally engaged in a patient-centric approach and immensely contributed.

Neuropsychological intervention helped with cognitive, emotional and behavioural outcomes that are often seen after brain injury. Along with medication for mood issues; behaviour modification techniques and cognitive retraining was carried out to facilitate his adjustment and functionality. The family including his wife and parents were regularly counselled and supported emotionally to cope with all the challenges of living with brain injury.

Over the past three to four months, he has made significant progress and is currently able to understand, speak, and communicate. His cognitive functions are improving except for mild memory impairment. He is able to sit, stand, and walk without any assistance and perform all daily tasks confidently.

Pravin was successfully discharged in December 2022 after undergoing what seemed like a herculean task. His cognitive, physical, and social skills had been remarkable improved. In addition to supervised gait training, fall prevention, functional ability training, memory training, and cognitive retraining, he continues to receive rehabilitation at the OPD after discharge. To prepare him for his return to his profession, specialists are working on advanced life skills training. This is a perfect example of comprehensive neurorehabilitation using a collaborative multidisciplinary approach which is globally recommended as the best evidence based practice.

Thanking the doctors at Sakra World Hospital, Pushpa, Pravin’s wife said, “Pravin’s accident shocked us, but what was even worse was when doctors had started giving up hope of his revival to his old self. I am grateful to the doctors and team at Sakra World Hospital who assessed his condition rightfully and took up his case, carefully designed his rehabilitation program, involving us. It is no less than a miracle that Pravin is up on his feet, and we are hopeful that he will resume his work soon.”

73 hospitals in Karnataka implement 100% breastfeeding guidelines under Manyata; ARTIST expands capacity building of hospital staff to 17 states

Breastfeeding Week – Dr Hema Divakar calls for real action

Bengaluru: Emphasizing the importance of breastfeeding for the first six months of an infant’s life, ARTIST for Her (Asian Research & Training Institute for Skill Transfer), in line with recommendations of various international institutions, has been successful in building capacities of 73 hospitals in Karnataka under ‘Manyata’, an ambitious initiative that brings together local health practitioners to improve their ability to deliver better, respectful and quality care services to pregnant mothers during and after childbirth.

Observing Breastfeeding Week (August 1-7) is not just about creating awareness, but ensuring that breastfeeding becomes a protective and life-saving intervention by educating and training mothers. Working in this direction, ARTIST, has institutionalized the practice of educating and training mothers by imparting a whole module on breastfeeding to the staff of the 73 hospitals under Manyata in association with the Federation of Obstetric and Gynecological Societies of India (FOGSI).

Renowned healthcare leader Dr. Hema Divakar, Chair of well woman healthcare committee at FIGO (International Federation of Gynecology and Obstetrics) & Chairperson, ARTIST, said, “Early and timely breastfeeding in the first hour of life can have profound benefits for both mother and baby. Breastfeeding helps provide colostrum to the newborn; it prevents newborn infections such as pneumonia and diarrhea. Sustained breastfeeding helps mothers in reducing the risk of non-communicable diseases such as Type 2 diabetes, cancer, stroke, cardiovascular diseases, and obesity.” She was speaking today on “Think Breastfeeding First – A call to action for Obstetricians and Gynaecologists”, organized by the South Asian Federation of Obstetrics & Gynaecology (SAFOG).

In addition to Karnataka, ARTIST has been undertaking capacity building of hospitals under Manyata in 16 other states. “There are strong myths and barriers owing to which breastfeeding is not practiced properly. We must disseminate information on breastfeeding but educate mothers at every touchpoint in maternity hospitals. New mothers need to be convinced, trained, and guided; All the stakeholders of healthcare must join hands to ensure that breastfeeding is practiced, as advised,” added Dr. Hema, who is also the National Convener of Manyata.

According to Dr. Hema, there are several issues related to breastfeeding that need to be addressed effectively. For instance, mothers have to breastfeed newborns for 6 months, but not all organizations support 6 months of maternity leave for working women. Likewise, creating breastfeeding corners in public spaces is needed. “In addition, there is confusion about the storage of pumped breast milk. It is perfectly fine to store breast milk for use later. But there is not much education around this,” pointed out Dr. Hema.

There is also a dire need to set up breastmilk banks, which are still in a nascent stage in India. “There are some efforts, but we definitely need milk banks with contribution from new mothers. Babies, especially premature babies, will benefit immensely. But there is a cultural barrier to donating breastmilk. We need to overcome that by educating women, creating a network, which will collect and store it as prescribed,” added Dr. Hema.

Unless there is real action, observing “Breastfeeding Week” will not yield the desired results, as 40% of the newly born babies in India go without being breastfed in the first hour of birth due to ignorance about the significance of breastfeeding. According to FIGO, early initiation of breastfeeding works towards attaining the WHO Global Target of 2025 to increase the rate of exclusive breastfeeding in the first six months to 50%, reduce low birth weight by 30%, reduce stunting by 40% and reduce and maintain childhood wasting at less than 5%.

Despite the benefits of breastfeeding, many countries have fallen short of achieving optimal early initiation of breastfeeding in the first hour of life. Globally, three in five babies are not breastfed in the first hour of life, which is an issue in high, middle, and low-income countries, including India, said Dr. Hema, quoting FIGO.

FIRST TIME IN KARNATAKA UNITED HOSPITALS BANGALORE CONDUCTS AUTOLOGOUS CARTILAGE TRANSPLANT BY ARTHROSCOPY

  • A definite destination for treatment of sports injuries Bengaluru , July 23 , 2022 : Sportspersons of the city and other locations now have an integrated place for the treatment of sports injuries at the United Hospital Bangalore . A recent case of treatment for sports injuries is that of Ajay , a young footballer who injured his ankle during one of his football matches . After visiting various doctors , he consulted Dr Pradyumna R , Consultant Orthopaedician at United Hospital .
  • A magnetic resonance imaging ( MRI ) scan showed damaged cartilage in the ankle . After careful consideration , Dr Pradyumna R decided to do an Autologous Cartilage transplant by Arthroscopy – a minimally invasive surgery ( keyhole ) – to heal the damaged cartilage . This technique was done for the very first time in Karnataka at United Hospital Jayanagar Bangalore .
  • Dr Pradyumna R said that ” Articular cartilage lesions in the athletic population are observed increasingly , due to limited intrinsic healing capacity , leading to progressive pain and incomplete and improper healing over time . If left untreated , isolated cartilage lesions can lead to progressive chondrogenic loss or the whole cartilage being lost over time ”
  • Trained in Arthroscopy & Sports Medicine relating to Shoulder , Elbow & Ankle Arthroscopy from the pioneers of the field in Germany , Dr Pradyumna strives to help people regain mobility and lead a healthier life . He is himself a sports enthusiast and loves cycling , playing badminton & swimming in his free time . Professional athletes and sports hobbyists are prone to cartilage damage . This may be due to repetitive action particularly twisting , jumping , and deep knee bends or a sudden traumatic injury like forceful pulling or a direct impact .
  • Dr. Vikram Siddareddy , Founder & Managing Director of United Hospital explained , ” The technology uses the patient’s own cartilage cells collected from the patient’s joint and this tissue is mixed with the patient’s own protein rich part of the patient’s blood to form a gel – like substance , and this is filled into the defective areas . This develops into the new cartilage in a few weeks thereby and halting the preventing damage in the joints further progression of arthritis in the joints .
  • With just keyhole surgery the process was carried out for the patient as a single stage procedure . Ajay recovered well and is continuing the exercise and rehabilitation programme . He will be able to play his favourite sport again very soon ” , he further added .
  • ” Recently a great number of sports – related injuries are seen as people renew their sports activities after the COVID pandemic . United Hospital is one of the best – equipped hospitals in Bangalore in terms of infrastructure , equipment , operation theatres , trained staff , management that is forward – looking and dedicated to patient welfare , and India’s first Autologous Cartilage Transplant being conducted here , United Hospital Jayanagar is the destination for sports injury correction ” says Dr Shanthkumar Muruda , Executive Director and Head , Diagnostic and Wellness .
  • Cartilage defects in the joints are usually treated without any surgical procedure . Prolonged immobilization , rest etc. are the norm in India which prevents people from continuing with their sports activities because of pain , stiffness , and incomplete recovery in the affected joints .
  • ” At United Hospital the best techniques in the world are used to treat all sports related injuries . Autologous Cartilage Transplant is done in a single setting , uses keyhole surgery and is less costly and convenient for the patient as opposed to conventional ACI which needs two different surgeries six weeks apart , ” Dr Muruda added .
