Karnataka, India, 13 December 2019 – First Response Emergency programme, a collaboration between the Government of Karnataka, Singapore Health Services & Temasek Foundation, the largest public healthcare cluster in Singapore, concluded today at the event held at Hotel Hyatt Centric Bangalore. More than 370 healthcare professionals from Karnataka, India, are now equipped with enhanced specialist skills that will allow them to provide better emergency treatment to patients during life threatening medical emergencies.
The symposium bought together more than 150 delegates, including the Health Secretary of Karnataka, ministry health officials as well as hospital senior staff. Experts from Singapore and Karnataka were involved in the First Response Emergency Programme and shared their perspectives on topics in the area of pre-hospital emergency care.
Commenting on the occasion Shri Jawaid Akthar IAS., Principal Secretary to the Department of Health and Family Welfare, Govt. of Karnataka, said, “36 Specialist Doctors and 4 Staff Nurses of the Department, visited Singapore in May 2019 to witness and learn on the learnings and services given by Singapore Health Services on pre-hospital emergency healthcare. The training was conducted after detailed assessment by the Singapore Health Services Emergency Medicine Experts with well-structured class-room and hands-on training modules. This has given rise to 52 Master Trainers in the Department, who are bestowed the responsibility to train the remaining Doctors and Staff Nurses in their hospitals. The Department is also strengthening the Ambulance services of the Hospitals across the State”
Associate Professor Chua Yeow Leng, Group Director, International Collaboration Office, SingHealth, said, “SingHealth is privileged to have worked with the Government of Karnataka and Temasek Foundation on this programme to improve the timeliness and quality of emergency care in Karnataka. This three-year programme has provided learning opportunities for both healthcare teams to sharpen pre-hospital emergency care skills, bringing long-term benefits to our respective communities.”
Mr Benedict Cheong, Chief Executive of Temasek Foundation International, said, “By bringing together health care leaders from Karnataka, India, and Singapore to share and learn from each other, we hope that the programme has enhanced capabilities in the area of emergency care and medicine for both communities. Beyond that, we hope that the connections formed during the programme will endure and lead to more meaningful collaborations between Karnataka and Singapore in the future”.
Ms. Vijaya Rao, Senior Director – Programme Development International Collaboration Office, SingHealth, said, “Overseas training programmes are great opportunities when it comes to Emergency Care, as it paves the way for mutual learning and gathering of knowledge. With this being said, our counterparts in India can gain clinical knowledge and develop their skills that will save lives of those in need. Through the alliance now established between Karnataka and Singapore, we at SingHealth intend to adopt a low-cost approach in order to counter rising healthcare costs, as well”.
Launched in 2016, the programme was designed to equip healthcare professionals in Karnataka with advanced emergency care skills to effectively manage emergency patients within the critical ‘Golden Hour’ (the period of time following a traumatic injury) to improve survival outcomes.
As part of the programme, the participants were also trained in Basic Cardiac Life Support so that they can provide timely treatment during cardiac emergencies. A select group of more than 50 healthcare professionals also underwent additional Advanced Cardiac Life Support (ACLS) training, making them the pioneer group of healthcare professionals in the public health sector of Karnataka to be trained in ACLS. With ACLS training, participants will have advanced medical skills that will allow them to provide emergency treatment during life threatening medical emergencies such as severe cardiac arrests situations.
A team of healthcare professionals from SingHealth led by Professor V. Anantharaman, Senior Consultant, Department of Emergency Medicine, Singapore General Hospital, conducted skills training workshops in key topic areas such as Emergency Critical Care and Protocols, Emergency Resuscitation Care and Emergency Medical and Nursing Care.
In May 2019, 30 people participated in the program in which 10 were selected as Master Trainers to undergo a one-week training at the Singapore General Hospital to gain further insights into Singapore’s emergency care management system. Together with ten healthcare leaders from the State Institute of Health & Family Welfare, they also visited Singapore Red Cross and the Singapore Civil Defence Force (SCDF) to observe community first aid teaching methodologies and the SCDF’s Emergency Medical Services protocols. On returning to Bangalore, the Master Trainers had 100 Emergency Medical Technicians trained to equip them with the necessary skills to manage and respond to calls from the public.
With the successful completion of the programme, the Master Trainers will go on to train more specialists in Karnataka to multiply the benefits of the training.
Approximately, 15 million people worldwide have suffered from stroke every year. In India, stroke has emerged as the leading cause of long-term disability and death and at 1.7 million per year has already reached epidemic proportions. According to the World stroke Organization, every 1 in 4 individuals is at a risk of developing this deadly disease.
Understanding stroke
A stroke occurs when blood flow to the brain is disrupted through either a blocked blood vessel (ischemic stroke) or a ruptured or burst blood vessel (hemorrhagic stroke) in the brain. 85% cases of stroke are ischemic in nature. A stroke can lead to loss of brain or bodily function or even death, depending on its severity and the part of the brain affected. Motor function, speech and memory are examples of bodily functions that may get impacted.
Stroke is a painless disorder. As there are no dire signs as opposed to a heart attack, it is difficult to notice and understand the emergency of stroke. Brain is an unforgiving organ and cannot regenerate itself unlike other organs such as kidneys, liver etc. A stroke can cause a person to lose nearly 2 million neurons every minute.
Lack of awareness – A challenge
Despite the vast number, stroke cases in India still go un-reported. Majority of stroke cases are not reported due to lack of cognizance about stroke or on the treatable options. Stroke in young population has become very common these days, especially in the age group of 30-40 years. As India becomes the capital of Non-Communicable Diseases with improper dietary habits, sedentary lifestyle, smoking and obesity, there continues to be a surge in stroke cases as well. These lifestyle-based risk factors are very commonly observed in metro cities.
It is critical for a person to reach a stroke-ready hospital within 6 hours of the incident. Unfortunately, only 1% patients reach within this “golden period”. The rest do not reach on time rendering their case untreatable. Patients are taken to the local nursing homes where there is no CT Scan and Neurological facility available and lose time in detection of the condition rather than initiating treatment which is quite crucial.
Dr. Vikram Huded, Head of Interventional Neurology and Stroke, NH Institute of Neurosciences, Bangalore said,“Stroke in young population has become very common these days. People between the age group of 30-40 years are affected. With advanced technology, treatments like mechanical thrombectomy (stent retrievers with which large blood vessels can be opened) have proven to be beneficial for treating stroke up to 24 hours after the recent clinical trials. I strongly recommend individuals with risk factors for stroke like smoking, diabetes and hypertension to undergo carotid doppler tests during their regular health check-ups.”
The current technological advancements in medicine enable doctors to potentially reverse the paralytic condition occurred due to Ischemic stroke. Advancements such as mechanical thrombectomy (stent retrievers with which large vessels can be opened) has been a boon for stroke treatment. They have proven to be beneficial for treating stroke up to 24 hours after onset of symptoms as reported in recent clinical trials. The way forward is to streamline pre-hospital management by training of first responders and ambulance service providers on recognizing the signs of stroke and reduce treatment delay by ensuring these providers take the patients to the right hospital (stroke ready) in time. It is crucial for every individual to be aware of stroke ready hospitals in your vicinity and reach within the golden hour. This helps avoid a delay in treatment and a team of doctors can confirm stroke and initiate treatment.
With recent advances in medicine and technology, Stroke can be managed effectively with adherence to treatment and lifestyle changes.
