A successful Liver Transplant performed on the heaviest patient in South India

Bangalore February 12th ,2021: BGS Gleneagles Global Hospital successfully performs a complex Liver transplant on a 38-year-old morbidly obese patient, marking it to be the first in South India. The patient was suffering from liver disease called as Decompensated chronic liver disease (Liver cirrhosis) and was listed for cadaveric liver transplant for more than 3 months. With no other option in hand, a team of Transplant Surgeons, Transplant Anaesthetists & Intensivists, and Hepatologists decided to perform Liver Transplant on Mr Ravindra who weighed 160 Kg. The team of experts meticulously planned the surgery ruling out all the complications and saved the life of Mr Ravindra.
Patient, a middle-aged businessman from Kolkata, was readmitted to our hospital after he got a liver offer from a cadaveric donor on 8th January 2021 and was discharged after three weeks in a satisfactory condition.
Dr Sunil Shenvi, Consultant, Multi-organ Transplantation and HPB surgery, BGS Gleneagles Global Hospital said, “When Mr Ravindra reached out to us for his liver problem, he was very weak and required immediate medical attention. After evaluating him completely, we decided that he needs liver transplant. It was a difficult decision for us considering his weight, but we decided to go ahead. We planned everything meticulously well in advance and were geared up to transplant the heaviest patient in South India”

Obesity has become the most significant public health problem worldwide. Many health experts have been witnessing surge in the cases of Non-alcoholic fatty liver disease (NAFLD) and Non-alcoholic Steatohepatitis (NASH) in India which are common liver problems in obese patients. Poor diet and sedentary lifestyle have taken a toll on the health of young adults which can progress to Liver cirrhosis. When the fat gets accumulated in the liver cells, gradually it causes inflammation and eventually leads to NASH (Non-alcoholic Steatohepatitis). Inadequate medical intervention during this phase can lead to scarring in liver causing End Stage Liver Disease.
Unlike any Liver transplant, this case was more unique and complex in nature, as the patient weighed way more than usual. The instruments, facilities and the treatment plan for the patient was different from the traditional treatment method. For instance, he was given nursing care in special bed particularly used for obese patients. Before his surgery, doctors wanted to enroll him in weight reduction programme or get bariatric surgery done; however, due to his liver problem it was extremely difficult to lose weight. Hence, doctors went ahead with the transplant as soon as a cadaver donor was available for transplant.
Dr Venugopal B Pillai, HOD and Senior Consultant, Liver Transplant, BGS Gleneagles Global Hospital said, “As the patient weighed 160 kg at the time of admission and also had liver cirrhosis, the team was well prepared to face any complications during the surgery. Initially, during the planning phase the team decided to get bariatric surgery (weight loss surgery) simultaneously, but the patients’ health condition was feeble at the end of the surgery, so we deferred it.”

As the transplant was complex in nature, it took 12 hours to perform the Liver transplant. Later, the patient was given post-op care in the Transplant ICU, followed by optimal nutritional therapy and best physiotherapy care to bring him back to normalcy.
Mr Sandeep Kumar, CEO – BGS Gleneagles Global Hospital said, “Being a pioneer of liver transplant, our doctors are fully capable to handle any complex liver transplant patient. I would like to congratulate the team for successfully performing the surgery. I would also like to thank the donor’s family for their selfless gesture and giving the patient a new lease of life.”

The team that spearheaded this complex surgery includes Prof. Dr. Venugopal B Pillai, Dr. Sunil Shenvi, Dr. Uma, from the Anaesthesia and critical care department, Dr. Kuttappa, Dr. Arun, Dr. Vijay, Dr. Ashish, and Dr. Mahesh, from Medical Gastroenterology and Hepatology, Dr. Adarsh, from Interventional radiology, Dr. Uttappa and Dr. Rohit, from Physiotherapy Mr. Mohan Babu, who made this surgery a success.

Mr Ravindra is currently out of danger and is continuing his physiotherapy care to reduce his weight and to avoid any further health problems.

CMR University opens Alternate Dispute Resolution Centre for students

CMR University opens Alternate Dispute ResolutionCentre for studentsTraining in Lok Adalat, Arbitration, Mediation, Conciliation, Mediation-Arbitration

BENGALURU: In an effort to promote Alternate Dispute Resolution practices, the CMR University School of Legal Studies inaugurated a Centre for Alternate Dispute Resolution (ADR) at its campus in Bengaluru. The ADR Centre will equip young law students in the practice of various ADR mechanisms, such as Training in Lok Adalat, Arbitration, Mediation, Conciliation, and Mediation-Arbitration, and counselling services.

The establishment of the ADR centre is yet another attempt of CMR University and CMR University School of Legal Studies in contributing to societal and community welfare. The centre will not only become a hub of research in knowledge related to ADRS, but also be a hub of activity in resolving disputes and offering simpler alternatives to the public.

The nation has almost 4 crore pending cases at different courts as per September 2020 data. Vacancy of judges has affected the disposal of cases. In 2012, the Supreme Court had approved the Scheme of National Court Management System to provide a framework for case management. The scheme estimated that with an increase in literacy, per capita income, and population, the number of new cases filed each year may go up to 15 crore over the next three decades, which will require at least 75,000 judges. Hence, there is an underlying need for alternative dispute resolution methods.

There is an underlining need for alternative dispute resolution methods. It is in this context that a Resolution was adopted by the Chief Ministers and the Chief Justices of States in a conference held in New Delhi on 4th December 1993 under the chairmanship of the then Prime Minister and presided over by the Chief Justice of India. Thus “The Arbitration and Conciliation Act, 1996” came into being. The advantage of ADR is that it is more flexible and avoids seeking recourse to the courts.

High Court judge Justice Shankar Ganapathi Pandit declared the ADR open in a virtual event and said, “It is a novel initiative to entrust the ADR Centre student committee the responsibility for the ADR activities. I wish every success for this venture.”

He quoted Abraham Lincoln. “discourage litigation, persuade your neighbours to compromise whenever you can, point out to them how a nominal winner is often a looser in fees, expenses and waste of time. As a peace maker the lawyer has a superior opportunity of being a good man. He said that ADR has undergone a phenomenal change, evolving from elders of the village sitting under a tree, solving differences to the stage of gaining statutory recognition.

Guest of honour Justice Budihal R.B, former judge of High Court of Karnataka, encouraged law students to specialize in specific fields like arbitration and mediation. Susheela Sarathi, designated Senior Counsel, High Court of Karnataka, said the university, through this initiative, stands out, and remains the Harvard of all Karnataka Universities in ADR. She also advised students to undergo training to enhance their communication, negotiation and other techniques.

Bariatric Surgery for Morbid Obesity Gives New Lease of Life to a Short-Statured Man 4.5 ft in Height

  • The 67-year-old patient of short stature had double his ideal weight and suffered from a range of health problems, including difficulty in walking and breathing
  • Surgery was considered a very high-risk option and many hospitals in Bengaluru turned away the patient
  • At Vikram Hospital, the surgeons successfully performed the surgery, with the patient managing to walk just three hours after the procedure

BENGALURU / February 12, 2021: A very challenging and risky bariatric surgery conducted by surgeons of Vikram Hospital, Bengaluru, has given a new lease of life to a 67-year-old patient of short stature who was suffering from morbid obesity. Weighing 77 kg, the patient Shekhar Krishnan had double his ideal weight, resulting in a BMI of 39 due to his short height of just 137 cm (4 feet 5 inches).

The condition led to several cardiac and pulmonary problems, knee pain, obstructive sleep apnea, elevated creatinine levels, and breathing difficulties as the fat burden pushed against his diaphragm. All efforts on the patient’s part to reduce his weight were in vain, and he found it difficult to conduct day to day activities due to the high visceral fat.

Said Dr. G Moinoddin, Consultant Bariatric & Advanced Laparoscopic Surgeon, Vikram Hospital, Bengaluru: “The patient was suffering from morbid obesity and metabolic syndrome since the last five years, leading to serious health issues. He was unable to sleep properly and even sit or stand. Walking just a few steps left him gasping for breath. Surgery was the only option to improve the patient’s quality of life and get relief from other comorbidities, but this was considered very risky due to his cardio-pulmonary condition. The patient was turned away by many hospitals in the city. If left untreated, the condition would have eventually led to cardiac arrest and death.”

The bariatric surgery conducted by Dr. G Moinoddin and his team was uneventful and lasted less than half an hour. Said Dr. Satish Thammanna, Consultant Anaesthesiologist, Vikram Hospital, Bengaluru: “The patient was able to walk just three hours after surgery. In less than a week since being operated upon, he has already lost five kilos. His walking has become better and shortness of breath has reduced. He is expected to lose another 30 to 35 kgs in the coming 8 to 10 months, bringing his BMI back to normal range. He will then be able to lead a normal life.”

Dr. Somesh Mittal, MD & CEO, Vikram Hospital, Bengaluru said, “We are quite elated at the successful outcome of the surgery because this is the first-ever case of its kind in Bengaluru. The patient was quite short in height, which posed a challenge for anesthesiology. Yet our doctors managed to achieve this remarkable feat when other hospitals turned away the patient due to the high risks involved. Our hospital is fully equipped and capable of carrying out such kind of challenging surgeries.”
