Varicose Veins: Causes, Symptoms, and Treatment Options

Dr A Arun Kumar, Senior Specialist – Vascular and Endovascular Surgery, Aster RV Hospital

Varicose veins are a common condition that affects the veins, usually in the legs, causing them to become enlarged, twisted, and bulging. They occur when the valves within the veins fail to function properly, leading to blood pooling and increased pressure within the affected veins.
While varicose veins are often considered a cosmetic concern, they can also cause discomfort, pain, and other complications. Fortunately, various treatment options are available to manage and treat varicose veins effectively.
Causes of Varicose Veins
Age and Genetics
As we age, the natural wear and tear on vein valves can lead to their dysfunction. Additionally, a family history of varicose veins increases the likelihood of developing this condition.

Prolonged Standing or Sitting
Occupations that require long periods of standing or sitting can put additional pressure on the veins, hindering blood flow and increasing the risk of varicose veins.

Hormonal Changes
Hormonal fluctuations during pregnancy and menopause can weaken vein walls and valves, making women more susceptible to varicose veins.

Obesity
Excess weight puts added pressure on the veins, impairing their ability to transport blood efficiently and potentially leading to the development of varicose veins.
When is procedure for Varicose Vein required?
Treatment for varicose veins is typically recommended when the symptoms and complications associated with them become bothersome or when they significantly impact an individual’s quality of life. Some common indications for varicose vein procedures include:
Pain and Discomfort
When varicose veins cause persistent pain, throbbing, aching, or cramping in the legs, treatment may be advised to alleviate these symptoms.

Swelling and Inflammation
Varicose veins can lead to swelling, inflammation, and a feeling of heaviness in the affected leg. Treatment may be considered to reduce these symptoms.
Skin Changes
Skin discoloration, rashes, dryness, and itchiness around the varicose veins can indicate the need for treatment to prevent further skin complications.

Ulcers and Bleeding
In rare cases, varicose veins can develop ulcers or skin sores, which are often difficult to heal. Additionally, if the veins are prone to bleeding, medical intervention may be required.

Treatment options for Varicose Veins
Several treatment options are available for varicose veins, depending on the severity and specific needs of the patient. These procedures aim to relieve symptoms, improve appearance, and prevent complications. Here are some commonly used treatments:
Lifestyle Changes
Simple lifestyle modifications such as regular exercise, maintaining a healthy weight, avoiding prolonged periods of standing or sitting, and elevating the legs can alleviate symptoms and prevent further progression.

Compression Stockings
Wearing compression stockings provides external pressure that helps improve blood circulation and reduces discomfort associated with varicose veins.

Endo-venous Laser Ablation (EVLA)
EVLA is a minimally invasive treatment that uses laser energy to heat and seal off the affected vein. It is effective for larger varicose veins can be performed under local anesthesia.

Radiofrequency Ablation (RFA)
Similar to EVLA, RFA involves using heat energy to close off the damaged vein. A catheter with a radiofrequency device is inserted into the vein to achieve this.

Ambulatory Phlebectomy
This surgical procedure involves making tiny incisions over the varicose veins and removing them through these small cuts. Ambulatory phlebectomy is suitable for larger varicose veins close to the surface of the skin.

Sclerotherapy
This procedure involves injecting a solution directly into the affected vein, causing it to close and eventually fade away. Sclerotherapy is commonly used for smaller varicose veins and spider veins.

Remember, maintaining a healthy lifestyle, including regular exercise, maintaining a healthy weight, and avoiding prolonged periods of inactivity, can play a significant role in preventing or managing varicose veins. Taking proactive steps to care for your vascular health can go a long way in ensuring your overall well-being and quality of life

Understanding the nuances of neuropathy

Dr. Keni Ravish Rajiv, Consultant Neurologist & Epileptologist, Aster RV Hospital

Peripheral neuropathy, also known as nerve damage, is a common condition that affects everybody at some point in their life and is characterized by the hallmark symptoms of tingling, numbness, pain, and muscular weakness.
Several types of nerve injury fall under the umbrella category of neuropathy. The number of affected nerves can be used to classify all neuropathies; mononeuropathies impact only one nerve, whereas polyneuropathies affect many. They are further divided into categories based on the kind of nerves (motor, sensory, or autonomic) they mostly influence.
Basics of peripheral neuropathy
Your peripheral nervous system (PNS) is a critical component of almost every bodily function, from sensory perception and voluntary motor activities like muscle contraction to autonomic processes like digestion, body temperature control, heart rate, and blood pressure.
Damaged peripheral nerves make it difficult for them to send and receive complete, coherent messages to and from your central nervous system. Physical trauma, an underlying medical condition, a vitamin deficiency, or exposure to poisonous substances are just a few of the many causes of harm.
These nerves may deliver messages that aren’t correct, like static on a phone line, or they may give signals that are warped, like a wavering television image. They might even completely stop signaling, like a damaged line. In injury-induced neuropathy, nerve damage can appear suddenly, as is frequently the case, or it can appear gradually and worsen over time, as is frequently the case with diabetic neuropathy.
Forms of neuropathy and symptoms
Although the type and degree of the symptoms that neuropathy causes are frequently used to characterize it, the first classification of neuropathy is based on how many nerves it affects. There are two primary categories of neuropathy:
Mononeuropathy
One nerve or set of nerves is affected by mononeuropathy. This type of neuropathy can develop from prolonged nerve pressure brought on by spending too much time sitting in a chair or lying in bed, however it is frequently brought on by physical trauma or injury.
Mononeuropathy can also occur as a result of persistent or repetitive movements, local compression, and chronic inflammation. The most prevalent type of mononeuropathy, carpal tunnel syndrome, generally manifests in just this way.
Symptoms typically manifest locally and widely. When it affects the peroneal nerve below your knee, you may have lower limb weakness that makes it difficult to lift your foot. When it affects the ulnar nerve in your elbow, you may suffer numbness in your elbow and tingling in your ring and pinky fingers radiating from your elbow.
Polyneuropathy
The majority of the time, neuropathy affects many nerves and nerve types, leading to extensive peripheral nervous system dysfunction. This disorder, sometimes referred to as polyneuropathy, typically develops when several peripheral nerves break down simultaneously due to an underlying medical condition.
Depending on the kind, location, and extent of the affected nerves, polyneuropathy symptoms can range from moderate and sporadic to severe and ongoing. Your reflexes and movement might be impacted by injury to your motor nerves, whereas your sensations and sense of balance can be impacted by damage to your sensory nerves.
An erratic heartbeat, excessive perspiration, bowel and bladder issues, enduring gastrointestinal symptoms, and abrupt blood pressure swings that make you feel dizzy or lightheaded are all indicators of autonomic nerve injury.
Options for treating neuropathy
As the majority of peripheral neuropathies are incurable or irreversible, treatment focuses on slowing or halting disease development and delivering long-lasting symptom relief so you can resume your normal, active lifestyle.
Keep your underlying medical condition under control if it’s diabetes that’s the root of your problem if you want to avoid more nerve damage.
The majority of neuropathy cases react effectively to specific lifestyle adjustments; many patients discover that maintaining an active lifestyle, eating a healthy diet, and decreasing weight can all significantly reduce the severity of their neuropathy symptoms. Mechanical devices like braces, occupational therapy, and physical therapy are all beneficial.

CITY MAN UNDERGOES A CRITICAL BILATERAL LUNG TRANSPLANT AT MGM HEALTHCARE IN CHENNAI

Bengaluru, May 19, 2023: MGM Healthcare, a multi-specialty quaternary care hospital located in the heart of Chennal, has recently achieved a significant milestone by successfully performing the BLT procedure on Mr. Madhusudhanan, a 61-year-old male diagnosed with Interstitial Lung Disease (ILD). The surgery was performed by a team of doctors including Dr. K. R. Balakrishnan, Dr. K.G. Suresh Rao and Dr. Apar Jindal.

The patient was suffering from Chronic Obstructive Pulmonary disease over a long period of time. The inflammation led to Interstitial Lung Disease (ILD) that caused subsequent scarring of the walls of the air sacs and reduced lung capacity and function. The patient was evaluated by experts at MGM Healthcare and was added to the transplant list. He was awaiting a suitable donor for two months and remained on oxygen support.

Speaking about the transplant, Dr. K R Balakrishnan, Director – Institute of Heart and Lung Transplant & Mechanical Circulatory Support, MGM Healthcare said, “The transplant was medically complex and required optimisation and careful planning. Organ transplant procedures require an expert team of surgeons, anesthetists, nurses, perfusionists, intensivists, transplant-specific physicians, and other multi-disciplinary team right from pre- operative to post-operative care. Right from diagnosis, deciding on the need and timing of the transplant, preparing the individual for the surgical procedure, during the procedure up to discharge from the hospital and long-term surveillance, represent a complex journey where every step needs meticulous care and expertise.”

Dr Suresh Rao KG, Co-Director, Institute of Heart and Lung Transplant & Mechanical Circulatory Support, MGM Healthcare, said, “The bilateral lung transplantation is a high-risk procedure. In heart and lung transplants, regardless of the blood group and tissue typing, sizing is very important. The donor’s lungs should fit the size of the chest cavity of the recipient. Lungs tend to shrink. We often cannot use donor lungs because of size mismatch.

In this case too, they were a little over sized but we waited and allowed them to adjust to the recipient’s cavity.”

Commenting on the procedure, Dr. Apar Jindal, Clinical Director & Consultant, Department of Lung Transplant & Interventional Pulmonology said, “The patient was presented with shortness of breath, fatigue, and weight loss. Detailed study and diagnosis revealed irreversible scarring of the lungs. ILD is an umbrella term used for a large group of diseases that cause scarring (fibrosis) of the lungs, leading to stiffness in the lungs which makes it difficult to breathe. Lung damage from ILDS is often irreversible and tends to worsen with time. Lung transplant was the best suitable option in this case and hence he underwent a bilateral lung transplant which has greatly improved his quality of life.”

Post the transplant, the patient’s hemodynamic gradually improved and the tracheostomy tube weaned off and he was able to breath normally in room temperature. The patient was discharged and is leading a normal life.

Dirty and Impure’ – Whisper aims to break these taboos around periods to help keep girls in school

1 out of 5 girls in India drop out of school due to lack of period education and products. Mothers are a girl’s first teacher, but reports say 7 out of 10 mothers do not fully understand the science of periods and find it ‘dirty or impure’.

Over the years, Whisper has provided free pads and menstrual education to 10 crore+ daughters and mothers. With every Whisper Ultra pack bought during this campaign, you can help support one girl’s free pads and menstrual education, helping keep her in school.

Bengaluru,March 13, 2023 – Studies show, 1 in 5 girls drop out of school every year in India due to lack of period education. Studies have also found that 7 in 10 mothers don’t know the biology of periods and consider it ‘dirty or impure’, often passing this to their daughters. With mothers being the first teachers of their children, it is important they educate their young daughters correctly about the natural process of periods and about using the right products.

To educate these mothers and to break the cycle of ignorance and misinformation, feminine-care brand Whisper, from the house of Procter & Gamble, has launched the fourth edition of its #KeepGirlsInSchool campaign. In a new film, titled, The Missing Chapter conceptualized by Leo Burnett, Whisper is reaching out to mothers, teaching them the biology of periods so that they, in turn, can educate their daughters on menstrual hygiene and using sanitary pads during their monthly cycles. In most parts of India, mothers themselves are unaware about the biological workings of their body during menstruation leaving 71% of the girls in India not aware about periods when they first get them. As a result, young girls have limited knowledge on how to manage their periods and prefer to stay home during period days every month.

Whisper launched its #KeepGirlsInSchool Campaign in the year 2020 to shed light on ‘Missing Girls’ from Indian schools every month. They continued with the thought-provoking campaign in 2021 highlighting, ‘Missing Future’ of girls who dropped out of schools during their periods. Most importantly, over the last 30 years, Whisper has been running its school and community programs where it has educated and given free pads to over 10 crore girls and to mothers.
In 2022, the brand brought to notice the ‘The Missing Chapter’ on periods in the school syllabi highlighting an important question – how long a Missing Chapter can be kept away from the Indian education system. Whisper also stepped up to its commitment of bringing period education to millions of girls who lost access to this education through the pandemic by stepping up the on-ground practice of building awareness about menstrual hygiene and period education.
Over the years, Whisper has provided free pads and menstrual education to more than 10 crore daughters and mothers. With every Whisper Ultra pack bought during this campaign, you can help support one girl’s free pads and menstrual education, helping keep her in school.

“For almost three decades now, we, at Whisper, have been dedicated towards period education, spreading awareness about menstrual hygiene, and distributing sanitary pads to girls across India. This year, we’re aiming to educate mothers, the girl’s first teacher, about the biology of periods and right sanitary products. Something that she was never taught herself. Our aim is to empower young girls to achieve their dreams by completing their school education without any gaps and to stop them from dropping out of school because of a biological process as natural as periods. Our consumers can make a huge difference in helping us bridge this gap. For every pack of Whisper Ultra sanitary napkins bought, we will give menstrual education and free pads to one girl helping keep her in school,” said Girish Kalyanaraman – Vice President and Category Leader, Feminine Care, Procter & Gamble India.

Sharing her support for the movement, actor Mrunal Thakur, said, “Menstrual education is so important to teach girls about their periods, their body and how to manage it. Whisper is doing an incredible job with its #KeepGirlsInSchool Campaign, and I am proud to be associated with it. I have been using their products from the start and still remember the session I had in school where the Whisper team taught us about periods. That helped me and my friends be ready for this change and embrace it as a natural biological process. It’s time to have open conversations about periods and create a world where menstruation is not a barrier to girls’ education and well-being. By keeping girls in school, we are not only investing in their individual growth and success but also in the progress and prosperity of our society.”

The KGIS campaign addresses all girls in India, breaking myths about periods being ‘dirty’ or an ‘illness’ by educating girls about the science of the female reproductive system and how the usage of sanitary products can help them go to school.
