World Sickle Cell Day 2022: Stem Cell Transplant the only curative treatment for Sickle cell Disease

Bangalore, June 16,2022 : There are as many as 2.5 million carrier of the gene (Hemoglobin AS) in India that can lead to sickle cell disease, with more than 1,25,000 actual patients spread across the country, with a much higher incidence in the tribal belt of the country. Sickle cell disease is associated with a significant risk of morbidity and premature mortality, especially among children. While in the west, a child receiving comprehensive care in high-resource settings has an estimated 99% survival into adulthood. However, in India, according to an ICMR study, about 20 per cent of children with sickle disease died by the age of two, and 30 per cent children with Sickle Cell Disease die before they reach adulthood.

Dr. Biju George, Professor & Head, Department of Haematology at CMC Vellore, “People with Sickle Cell Disease, may beat a risk of progressive organ damage, impaired quality of life, considerable morbidity in childhood, and risk of premature mortality in adulthood [median survival of 58 years]. Sickle Cell disease patients who are undergoing regular life-long blood transfusions, have the best chance of survival and cure with a blood stem cell transplant. This transplant can come from a sibling or a family member. However, there is only a 30% chance of finding a “matched” sibling donor in the same family. The remaining 70% patients look for a matching donor through a stem cell registry or donor center- a database of voluntary donors between the age group of 18 to 50 years.”

Dr. Govind Eriat Nair, Consultant Hematology Hemato-Oncology and Bone Marrow Transplant Gleneagles Global BGS Hospital, Bengaluru, “If there is a fully HLA matched donor in the family, there is a 90-95% chance of cure with higher cure rates in younger children of below 12 years of age. However, due to underrepresentation of Indians in the global donor data pool, patients are unable to find a match on time. Also, the awareness about the disease is less, need of the hour is to raise the social awareness about this disease. Genetic counseling and newborn screening are the way forward. Effects of endogamy, consanguinity and role prenatal counseling needs to be addressed in primary screening.”

DKMS BMST Foundation India is a non-profit organization dedicated to the fight against blood cancer and other blood disorders, such as thalassemia and sickle cell disease. The organization aims to give every blood disorder patient in need a second chance at life.

To mark World Sickle Cell Day, Patrick Paul, CEO, DKMS BMST Foundation India said, “Sickle cell has variance and only the severe form needs a stem cell transplant. An early transplant can help patient with severe Sickle cell disease from organ damage. With rising cases in India, it is the need of the hour that stem cell transplants are made available to more patients to save lives. But due to the misconceptions and lack of awareness about blood stem cell donation, Indians are highly underrepresented in the global donor pool. This situation can only be changed by recruiting many more potential blood stem cell donors from the Indian ethnicity.”

The success of a stem cell transplant depends of donor’s HLA (Human Leukocyte Antigen) matching the patient. The body’s immune system has proteins known as HLA to distinguish cells that belong to the body from those that do not. DKMS-BMST helps in unrelated donor transplant process which includes enrolling and counselling the donors, get their HLA typing done, facilitate search of the donors and later facilitate the blood stem cell collection and the transplant. So, far DKMS-BMST has registered over 60,000 potential donors and have helped 60 patients with second chance at life.

Register as a potential blood stem cell donor:

Healthy individuals between 18-50 years of age can register at: dkms-bmst.org/register

All it takes is five minutes of your time and a simple 3 step process:

Step 1: Visit the site, fill up an online form and you will receive a DIY swab kit at home.

Step 2: Once you receive the swab kit, fill out the consent form and take a tissue sample from the inside of your cheeks with 3 cotton swabs provided in the kit.

Step 3: Send back your swab sample in the pre-paid envelope provided.

DKMS laboratory will then analyze your tissue type and your details will be available in the global search for blood stem cell donors. If you do come up as a suitable donor, DKMS-BMST will get in touch with you straight away. Once you come up as a match, blood stem cells will be obtained from the bloodstream using a procedure called Peripheral Blood Stem Cell Collection, which is similar to a blood donation wherein only your stem cells are taken. This is a safe, non-surgical outpatient procedure.

Men have higher mortality rate than women, yet remain in denial about health and ignore symptoms

BENGALURU / JUNE 16, 2022 : In 2019, the mortality rate for women was 145 per 1,000 female adults while that for men was 201 per 1,000 male adults in India, yet men are often not attentive towards their physical wellbeing due to social conditioning. This impacts their health adversely, leading to a shorter lifespan than women. This was said by doctors of NURA, a collaboration between Fujifilm Healthcare and Dr Kutty’s Healthcare offering AI-enabled imaging and expert healthcare at Bengaluru, in the ongoing Men’s Health Week.

Said Dr. Tausif Ahmed Thangalvadi, Medical Director, NURA, Bengaluru: “There is a general tendency in men to think that if they are working productively and living up to their role in society, they are fit and healthy. Due to this, they avoid taking care of their health proactively. They consult doctors less frequently compared to females and reach out for medical help only when symptoms turn severe, which puts them at higher risk of critical ailments.”

He added: “There are five barriers that stop men from seeking help. The first is denial. They assume that small symptoms can be ignored. The second is that they consider seeking help as a sign of weakness. The third is that they retain a provider image of themselves. Taking care of family or loved ones takes precedence over taking care of their own health. The fourth is that men find it awkward to discuss their health with family and friends. Lastly, they often lack awareness about where to get the right information about their health.”

Common health threats to men include heart attacks and strokes (the No. 1 killer disease for men), diabetes, prostate cancer, lung diseases like COPD and lung cancer due to smoking, depression, and sexually transmitted infections.

Said Dr. Tausif Ahmed Thangalvadi: “Our screening of around 2,000 healthy and asymptomatic men over the last one year in Bengaluru has thrown up interesting data. About 70% of men had excess visceral fat, which leads to higher risk of heart disease and diabetes. About 35% were found to be diabetic, while another 20% were prediabetics. Over 25% of asymptomatic men with no previous history of heart disease were found to have calcium deposits in their coronaries, which increases the risk of heart attacks. About 30% of men screened had abnormal cholesterol levels. Nearly 5-6% had suspicious lung lesions which needed further opinion from a lung specialist. On the brighter side, Bengaluru men seem to maintain good oral health, as oral cancer lesions have been rarely found at our centre during screening.”

He added: “The majority of underlying causes of higher morbidity rates in males over females relate to modifiable and preventable lifestyle choices made by men. The epidemic of obesity and diabetes is directly related to smoking, poor diet, excessive alcohol consumption, and sedentary lifestyle or lack of physical exercise. To improve their health and live longer, men need to adopt a healthy lifestyle with regular exercise and balanced diet, avoid smoking and tobacco, and go for regular health screening that also should include testing for cancer.”

According to Dr. Tausif, all men should undergo annual health screening foe at least eight parameters. These include heart health check, lung health screening, screening for oral cancer, colon cancer and prostate cancer, diabetes, blood pressure monitoring and lipid profile. “Regular health screening helps in early detection of critical ailments, making treatment timely and quite effective. It encourages people to have a healthy lifestyle and adopt better health habits, thereby reducing health risks,” he said.

BGS Gleneagles Global Hospital – first in Karnataka to touch the 250 Liver Transplant mark

● First to complete 250 liver transplants in Karnataka *First to perform adult swap liver transplant in Karnataka *First to perform Multi – Organ ( Kidney + Liver ) transplant in Karnataka *First to perform SWAP transplant in Karnataka during the COVID – 19 pondemic *Gleneagles Global Hospitals network is the pioneer in organ transplantation in India .

Bangalore , June 16 , 2022 : BGS Gleneagles Global Hospital is one of Karnataka’s leading multi – organ transplant centres . It has the largest integrated liver care program in Karnataka with a strong legacy of 12+ years . It has reached yet another milestone by crossing the 250+ transplant mark becoming the first hospital in Karnataka to complete 250 liver transplants . The hospital is well – known for its vigorous transplant programme and is Karnataka’s pioneer in more complex transplants such as combined liver – kidney , split liver and adult swap liver transplants . Despite COVID induced challenges , the hospital carried out 13 liver transplants in 2021. The team of liver transplant surgeons have transplanted 120 livers from deceased donors and 119 from living donors , as well 11 kidney – liver combinations . Even though it treats some of the most critical patients , BGS Gleneagles Global Hospital’s liver transplant outcomes are among the state’s best , with a one – year survival greater than 90 percent and a five – year survival greater than 85 percent . ”

The achievement is more than just a number it represents the lives of patients touched . by the generosity of organ donors and is also a reminder of the countless others on the waitlist who struggle with liver disease and are waiting patiently and desperately for precious , life – saving organs . ” said Dr Mahesh Gopasetty , HOD & Senior Consultant – HPB and Transplant Surgery , BGS Gleneagles Global Hospital One important way of increasing the number of available organs has been living – donor transplantation , a technique that enables the patient to obtain a transplant right away , skipping the waitlist . ” A living donor can be an immediate family member or extended family ” explains Dr Mahesh . ” In the procedure , a portion of the donor’s liver is removed and implanted in the recipient . Both segments ( the remaining section of the donor’s liver , and the portion received by the patient ) will regenerate and grow to fit the needs of each individual . ” At present , 70-80 % of livers obtained for transplantation in South India are a result of cadaver donations . A very small percentage is from living donors . The doctors anticipate that this imbalance will correct itself in the future as awareness increases about the fact that live donors need not fear any risk to life or longevity by deciding to donate . Last year , a child with Wilson’s disease was brought to the hospital with liver failure . Wilson’s disease is a rare inherited disorder that causes copper to accumulate in your liver , brain and other vital organs . As liver cells try to make repairs to damage done by excess copper , scar tissue forms in the liver , making it more difficult for the liver to function . The child was transplanted with a liver donated by her mother . In this case , the expense of the surgery was met through a combined sourcing strategy .

Apart from crowd funding , the hospital chose to award a discount and only the remaining amount had to be arranged by the family . Dr Adarsh CK , Chief Gastroenterologist and Hepatologist , BGS Gleneagles Global Hospital , ” The liver program at BGS Gleneagles Global Hospital uses the most advanced technology to treat all types of acute and chronic liver conditions , including treatment for hepatitis B & C , fatty liver disease , alcoholic cirrhosis , liver tumours and management of metabolic , autoimmune , and hereditary liver diseases . ” Another medically challenging case that the transplant team recently treated is of a young woman who was suffering from port pulmonary hypertension , a heart condition that is sometimes associated with liver cirrhosis . The patient had very high pulmonary artery pressure , which meant an increased risk of cardiac arrest during surgery . This condition also causes the body to hold onto excess fluid . Therefore , the patient was admitted two weeks prior to the transplant and treated aggressively to manage its severity before going into the surgery . Nearly 20 litres of fluid were removed from her body during this time . Her husband donated his liver , the surgery was successful and now she no longer suffers from port pulmonary hypertension either . This transplant made possible by first treating other related conditions which had severe consequences to her health . ” This major milestone is the result of the tireless dedication of everyone on our liver transplant team including hepatologists , surgeons , diagnostic and pathology experts , advanced – practice nurses , and others , ” says Biju Nair , COO , BGS Gleneagles Global Hospital ” Together we have come a long way in the last 12 years , pursuing innovations to increase access to transplantation and improve quality of life for patients with end – stage liver disease . ” The team of experts who made this achievement possible includes Dr Mahesh Gopasetty ( HOD & Senior Consultant – HPB and Transplant Surgery ) , Dr A M Kutappa ( HOD and Senior Consultant , Anaesthesia and Critical Care ) , Dr Adarsh CK ( Chief Gastroenterologist and Hepatologist ) , Dr Pradeep Krishna ( Consultant – HPB and Transplant Surgery ) , Dr Pramod ( Consultant Hepatologist & Liver Transplant Physician ) , Dr Ravindra Nidoni ( Consultant – HPB and Transplant Surgery ) , Dr Bhuvan Shetty ( Gastroenterologist and Hepatologist ) , doctors of the Anaesthesia and Critical Care team consisting of Dr Ashish , Dr Arun , Dr Vijay , Dr Mahesh , and Dr Uma , supported by a dedicated team of transplant nurses , coordinators and other supporting staff members . The hospital also has strong Laboratory team and Blood Bank equipped with facilities that are at par with international standards and helps in making transplant program successful . The COVID – 19 pandemic strongly affected organ procurement and transplantation in Karnataka . Non – availability of donors and travel restrictions further affected those who were immune compromised and awaiting scheduled liver transplants . Many patients unfortunately succumbed under these circumstances . Being home bound , many people . took to alcohol and binge eating which has also caused an increase in liver related health issues in the Bangalore population .
